  • PHYSICAL EXAM is required for school Entry/Kindergarten
  • Exams may be completed by your personal physician (forms available in links above) or completed at school by school physician and school dentist.
  • DENTAL EXAM is required for school Entry/Kindergarten and for 3rd-grade students
  • Exams may be completed by your personal physician (forms available in links above) or completed at school by school physician and school dentist.
  • IMMUNIZATIONS are required by the Pennsylvania Department of Health for all children entering school by the fifth day of school or the student could risk exclusion from school. Please see the Immunization Information link above for more details
  • Please remember to update phone numbers and emergency contacts in Skyward as they change throughout the school year to ensure we have the most current information on record on how to reach you in case of an emergency!
  • Please remember to update the nurse on changing health issues or new chronic diagnosis!
